While Mr Sureshot there has a good point and I at least partially agree, you can fast-track some of this... the question is what vices do you have now?...
Alcohol? Smoking? Drugs? (and other kinds of substance abuse, even coffee)....
Is your diet healthy, a Professional Nutritionist CAN help, but you need more than just a healthy diet PLAN you need to be able to follow it through
Exercise, do you do any? is it enough?... some is better than none, and depending on your goal ie to get big and bulky? to get strong and lean? there are too many options to say here just which ones are good or bad for you without more information...
Subtle differences can help, feel free to take babysteps, they're generally pretty easy and if you think of the ';swiss cheese'; method, the more holes you poke in it, the sooner theres nothing left to poke holes in!... also babysteps are easier to continue with once you are stable in that routine...
Any positive change in diet, exercise and lack of substance abuse, etc. no matter how small is going to help you feel better for longer...
As to mental health, well working on physical health goes along way to assisting with mental health, just keep your brain active with a puzzle like a sudoku or crossword each day... doesn't matter if you complete it or mess up or can't figure it out... just have a fair attempt at it each time and after a while you may discover something you hadn't thought of and eventually you'll be embarassed that there was a time where you couldn't do them!... Also they do tend to try to put arcane or unlikely words into crosswords in my experience so don't feel too bad...
Another way if that doesn't work for you is simply to read (or if you prefer write) something, doesn't have to be a novel, could be an article from a magazine, a paper or even read something from a study guide ...

Give up alcohol and smoking
You can never, ever, cold turkey on vice. After five and twenty years, you are set in your ways more than you know. The only thing that could snap you into a new routine is some deep shock or epiphany and even that wont last long.
What you have to do is slowly change and have good friends to support you along the way. You have to completely reject the people in your life that have led to your continual advancement in deterioration of your health and your mental imagine. Even though you have friends to support you, you must develop an identity by yourself, a strong identity that will keep you going for yourself without the need of anyone else. It all starts with looking into the mirror and saying 'I love you'. Without loving yourself first, nothing is possible.
Well...that's just my two cents, which I've aquired through personal hardship.
My father has always lived a very healthy and active life.
He is now 84 yrs old and has suffered a stroke.
He lives in an old peoples home where nurses feed him and massage him to prevent bed sores. He no longer goes to the toilet he wears a nappy. His speech is almost non existent and on the rare occasion he knows who I am.
The doctors have told me that because of his ';well spent youth'; he is saved from death.
Work it out for yourself, as for me I enjoy my life now while I know what I am doing.
Just like the Bible
Don't judge as to what the person eat
because it goes in from the mouth and out to its belly
but watch the thing that comes out to his mouth
because it came from the hearth and in his mind.

To be honest moderation is key.
Have a drink once in a while, eat a freasy burger or what ever your poison is, otherwise you'll end up bored and go mental. At the end of the day you could get run over by a bus tomorrow so you might aswell enjoy life.
